By accessing or using the services provided by True Vine Support Coordination, you agree to the following terms and conditions.
True Vine Support Coordination provides Support Coordination, Psychosocial Recovery Coaching and SIL Engagement services in accordance with relevant NDIS guidelines and professional standards. Services are tailored to individual participant needs and goals.
True Vine Support Coordination is currently not a registered NDIS provider. In accordance with NDIS guidelines, services are currently available to participants who are self-managed or plan-managed.
Participants are responsible for providing accurate information relevant to their supports, goals and circumstances. Participants are also responsible for ensuring sufficient funding is available within their NDIS plan for requested services.
Where applicable, a formal service agreement may be provided outlining supports, responsibilities, cancellation terms, pricing and service expectations.
We are committed to protecting your privacy and handling personal information in accordance with applicable Australian privacy laws. Participant information will remain confidential unless disclosure is required by law or authorised by the participant.
Service fees will be discussed and agreed upon prior to commencement of supports. Participants are responsible for payment of agreed services through their nominated NDIS funding arrangements.
We require 48 hours notice for cancellations or rescheduling of appointments. Short notice cancellations may incur fees in line with NDIS pricing arrangements where applicable.
While we aim to provide high-quality and professional support services, True Vine Support Coordination is not liable for decisions made by third-party providers, funding outcomes determined by the NDIA, or circumstances outside of our control.
We reserve the right to update, modify or discontinue services, policies or website content at any time without prior notice.
